DEADLINE

Health Insurance Enrollment ends this Friday, December 15th!

                   We can book your Annual BPQ Review Appointment if you do the following:
                   You must send your completed BPQ by Tuesday, December 12th to start
                   shopping for your new Health plan.

  • Be sure to complete your Doctor Search with the instructions shown at the top
    of Page 2 of your BPQ before emailing it.
  • Please Note: You will not be given an appointment without a completed BPQ.
    If you do not search for your doctors, we will assume you do not care to retain
    your providers and are open to all insurance company plans we recommend    .
  • Email your completed form to administration@benefitsandpercs.com.
  • Once we receive your completed BPQ we will contact you to set your first
    appointment with Melanie.

  • Use this link to access your Benefits and Percs Questionnaire (BPQ) Form:

  • After you download the form, type your details on the form, be as specific as possible.
  • Download the BPQ to your desktop.
  • Then save it titling it with your name followed by BPQ.
  • Be sure it is saved on your computer. We receive many blank BPQ forms.

How to Download and Save the BPQ
on Your Computer

  1. Click the BPQ link above
  2. BPQ will open
  3. Click on the “File” tab
  4. Click on “Save Page As”
  5. Title the document with your name followed by BPQ
  6. Save it to your desktop
  7. Click on “Save”
  8. You can now locate your BPQ on your desktop screen
  9. Email the completed BPQ form to administration@benefitsandpercs.com
